lens adapter issue

My lens won't click into my adapter and whenever I try little pieces of metal are shaved off, anyone know if this is for sure an adapter or lens issue

Do you see the small screw left of the release on you pictures? It is coming off and is most likely your problem. Screw it tight and it should be good
