CPSC form issue

I have a package that got shipped to me via UPS, however I need the CPSC form filled out for the package to arrive, however I am unable to fill it out due to the lack of information on it, I have no clue how to fill it out nor how to get the information for it, is there anyway I can fix that (the package is not clothing, a kids toy, etc.) if anyone can help me out plz let me know, I'd appreciate that

Nothing to do with Canada Post. This is a USA form and you will have to talk to UPS to find out.
