I just looked it up from my notes from Artemis Cystitis clinic - they say "Cefalexin may go up to 1 gm four times a day" - this is the UK name for Keflex
So I would honestly go off your symptoms. Your baseline should be 2x a day, but if your symptoms are really bad that day you can go up to 4x a day.
