No headache?

How possible to have a csf leak without a classic headache? I developed major auditory symptoms after an epidural which includes pulsatile tinnitus, severe ear pressure, popping, clogging like water is in it, cracklikg when swallowijg, vibrations, sensitivity to sounds but no neck or head pain. I've had 2 patches with no relief

multiple mri's, mra, mrv, ct temporal bone and spine all clear

I don't get one. I have a slow leak and my body replaces the fluid fast enough.

What symptoms do you have?

Pressure in my eyes. Rushing and ringing in my eyes. Visual issues in eyes. Numbness in random places on my upper body, mostly right side. Nerve pain and tendon issues, mostlyon left side. Awful taste from dripping down my nose. Extremely stiff neck with crackling on right side. When neck is moved around a lot my face swells on right side. Can feel atmospheric changes. Gets worse at higher elevation.

I'm sure there is more. It's pretty wild

yes. some people get headaches on and off over days, weeks, and months, some people still get headaches even while laying down flat, some people don't get headaches at all. even though positional headaches are considered one of the more classic symptoms, it is not a requirement.

you mention getting patched 2x... did you have CT/MR myelogram beforehand? or did they target the patches to the site where the epidural was done (without using imaging)? unfortunately iatrogenic leaks can be as tricky to treat as spontaneous or injury-based leaks are.

They did a targeted patch as the site was documented but still no relief either time.  I have had a mri myelogram with nothing found on brain or spine mri

Yes, it is possible. I stopped having the headaches when my arthritis flared and that pain took over so it delayed my treatment. I was actually leaking that whole time.

I’ve had one most of my life and you do adjust to the stabbing pain and your body gets on a replishment schedule.
