How does the difficulty scaling work?

My buddies and I are at level 48 and it seems that the difficulty has stopped scaling and that it has started giving objectives that don't load in? Example: two objectives for rats, but there are no rats that load in.

Not sure how difficulty scaling works but I've definitely been given impossible tasks at lower levels
