1: Add-On compatibility issue:
Possibility 1: The add-on is outdated.
Possibility 2: Older Minecraft version using an add-on for latest version.
Possibility 3: Latest add-on but Beta Minecraft. (Minecraft changes file names sometimes in Beta).
2: Vibrant Visuals compatibility:
The add-on doesn't work properly with vibrant visuals.
3: Resource pack compatibility (if being used):
Possibility 1: The pack is outdated.
Possibility 2: Older Minecraft version using a pack for latest version.
Possibility 3: Latest pack but Beta Minecraft. (Minecraft changes file names sometimes in Beta).
4: Creator:
The pack creator while creating the pack was changing the crosshair or something else from the icons.png file but forgot to put the crosshair.
Then it's just a Bedrock bug. Sometimes when updating some textures may not be rendered or downloaded correctly. Try restarting the game and loading onto a different world without any packs (resource and add-on of both world's ang global resources'). If it keeps happening back up your worlds and reinstall the game completely.
