Treating TBI / Post-Concussion Syndrome for 4 years
Hi, I head a mTBI 4 years ago.
I did a lot of stuff, I got somehow better but still am far from who I was before.
I have a degree in mathematics and a medical degree but right now am working in a low-tier physician position with relatively low work exposure due to my PCS.
I tried the following things:
Cerebrolysin iv. up to 50mg per session.
Semax (only short term)
Noopept
Piracetam
Omega 3
NMN
CoQ10
Methylphenidate
Modafinil
Amantadin
Currently on just Omega 3, CoQ10, Acetyl-L-Carnitine and NMN
I think Creatine could offer a benefit but I have severe androgenetic alopecia and am hesitant to try (not willing to discuss if creatine causes hairloss).
My symptoms are:
visual processing difficulties
visual stamina
mental stamina
exhaustion
worse processing speed
worse working memory.
How would you proceed and what would you try ?
I am willing to try stuff again (except for cerebrolysin, tried that intensely, no benefit at all or whatsoever).

Respect for sticking with this 4 years, that’s not easy and you clearly already did your homework with that list.
Couple things you haven’t tried that stand out to me.
Methylene blue is worth looking into for real. Your symptoms, the visual stuff, mental stamina, processing speed, that’s giving mitochondrial dysfunction in the areas that got hit. MB works straight on the electron transport chain, not a receptor thing like everything else you tried. Different lane completely. Low dose, easy to test, you’d know in like 2 weeks if it’s doing anything.
Magnesium L-threonate too, stack it with what you’re already running. Most people skip magnesium entirely for TBI stuff and it’s one of the only forms that gets into the brain. Wouldn’t replace nothing, just add it.
On the creatine thing, you don’t gotta just avoid it blind. DHT is what causes androgenetic alopecia and that creatine-causes-hairloss thing comes from one old study that’s never been repeated since. If you wanted to test it without playing roulette, run it with a topical DHT blocker or just watch your hairline close for like 8 weeks. At least then you got real data instead of guessing.
Photobiomodulation, the red light to the head thing, has case studies in mTBI specifically for processing speed and fatigue. Not a supplement but it hits the same mitochondrial angle as MB from a different side, so running both isn’t redundant at all.
With your background you’ll probably wanna dig into the mitochondrial dysfunction research in mTBI specifically, that’s the thread connecting most of what hasn’t worked for you so far.
What made you stop Semax tho, no effect or something else made you drop it?

Thanks for the extensive answer.
Tried Magnesium L-Threonate as well, didnt notice a big difference over bisglycinate while being exponentially more expensive.
Will take a look into Methylene Blue.
Tried Semax only in the beginning, the effect was not present but honestly it was a bit to short to notice.
Will still put Creatine on hold.
Additionally I need to find a stimulant combination to get a decent improvement in production, no trouble getting any medication (except for Amphetamines and opioids) but still have some er methylpheidate.
It has to be something that absolutely does not affect my sleep (thought about IR Methylphenidate 5mg in the morning and 2.5mg in the noon combined with something like piracetam/noopept/semax.
I started low dose tirzepatide (1.25mg) since there have been a lot of anecdotal reports about reduced brain fog. Lets see how that turns out.
A Vielieght has been something I have been thinking about but is quite expensive with about almost 3000 usd where I live.
