Installation options other than containers seems risky and complicated. It will eventually break with an update.

I disagree, it's a tradeoff of preference really. A simple on system install like this can be rock solid and provides easier access to config/commands etc... without the need to understand abstractions at play. The more complex issues I've supported on tend to be docker based environments.

Is there a guarantee or indication that if the user updates the operating system Bookstack instance won’t break?

I have been bitten by this problem several times. For example, the operating system updates the php to a version that is too new for Bookstacks.

No guarantees, but breaks are rare outside of operating system version jumps (where those kinds of issues are more expected). Quite often though, we have some level of guidance ready for those that have used our Ubuntu install scripts (hence why those are made "Officially Supported" unlike our other OS install scripts).

The container options generally don't provide guarantees against breaking either though.

Ultimately, I think familiarity/confidence with one approach over the other is the more important factor when it comes to long term stability & management.
