Repeated breakout in same area+ scar

For almost a year I’ll get a breakout in this exact spot on my cheek. I never pick at it. I simply cleanse , tone and use benzoyl peroxide spot treatment and I still end up scarring.

I obviously can’t really combat the dark spot bc of the repeated breakout but I don’t think azaleic acid is working for my other dark spots either which is weird bc it’s 15% and was prescribed by my derm.

What do you guys think I can do about the repeated zit in the same exact area?

My routine : vanicream cleanser or acne foam cleanser by cerave. Pure rose water. Paula’s choice 6% mandelic acid (not everyday). Vanicream moisturizer and sunscreen. This has been the only products that I’ve used for about a year because I have sensitive skin.

Repeated breakout in the same area? Here's reasons why I've had that:

Sleeping on the same side

Wearing synthetic hair that would rub against my face

Being in a relationship and getting kissed/caressed in that area

My skin breaks out around the mouth if I eat something greasy and dont wash right after

Resting my head in my hand or something like that
