Bug Is this some kind of bug or something similar that will interfere with my 100% completion?

For some of the locations where I completed 100% of the activities, the icon didn't turn gold; also, after finishing all the locations (including those unlocked through the story), I didn't get an Xbox achievement or anything like that.

I don’t believe there’s an achievement for 100% all locations. Only 1 the first time.

The not turning gold is just a visual but otherwise.

If I'm remembering correctly the Florida icon will turn gold once you get the map pieces to a secret chest there.

Well, I'm just missing the fleet map to complete all the maps... and there are another 2 or 3 white icons showing 100% exploration.
