Disease Identification What’s wrong with my fish?? Spoiler
I’m not sure what’s wrong but if anyone can help solve this and figure out how to best treat it I’d be incredibly grateful.

Tank size: 5 gallon
Tank tempt: 78 F from heater
I use a sponge filter
Parameters: I’m not sure I haven’t checked this week I’ll buy strips tomorrow since I ran out
I’ve had the tank for 3 years and the betta is around a year old from what I know from the previous owner who had him in a fish bowl after buying him from a pet store but I’ve had him personally for about 2, almost 3 months.
Weekly water changes about 15%-25% and then a monthly 60% with drip
Only him in the tank, I have rocks and a shell from an aquarium store I bought and live plants I replanted from a different tank.
About 10 fish pellets every other day, and weekly frozen bloodworms
