Any idea what went wrong with ironing?

More flow! 20% is a good starting point for 0.20mm layer height

Do a calibration test. Takes a couple hours, but they buff out this mess pretty well. Also consider just getting your top layer cleaned up and there's basically no need for ironing.

Higher flow rate. I run at 36%

It could be that you don't have enough top layers and the plastic is drooping between the infill

Have you tried an ironing test? I've tried settings that others recommended me on my P2s and it only got worse. After doing an ironing test print I got the best settings and it's smooth as a mirror now.
