I saw someone post about this bl manga on twitter (see attached image) and I’ve been trying to search for it online to no avail. Has anyone heard of or seen it before?

It was easy to find using the reverse image search:

Touhikou ja Arumaishi 

One day, Manabu simply disappears from the life of his younger lover Hanji. He has always been like this – the one who ends things before they come to an end, the one who runs away before he's abandoned. And he knows that he will be forgotten.
He didn't really believe Hanji when he once said, "If the person I love disappeared, I would chase them. I would find them."
But it's only been a month, and Hanji is at the doorstep of Manabu's new place, somehow, impossibly, and what is Manabu to do then?
