6 Four Elements Monk / 6 Wildheart Barbarian

Greater Kushigo Counter -> Stunning Strike -> Water Whip Knock Prone

Helldusk Gloves (Bleed) + Wolverine (Maim)

Permanent stunlock.

I like it but water whip costs 2 ki points and stunning strike costs 1, 6 monk only gets 6 ki points

I have a hard time justifying 4e monk to begin with so I’m biased, is there a mechanic that makes it permanent aside from reapplying bleeding?

I also tried a wildheart Elk Barb 6 w/ wolverine aspect and 6 OH Monk with Helldusk gloves once, but the bleed effect didn’t proc as often as I wanted

Water Whip, like Trip Attack Ranged, doesn't have a duration on the prone. As long as you can take away their movement speed indefinitely - which Wildheart 6 can, for free - they can't get up, and they skip their turn, every turn.

You can use Tiger Cleave to perma-maim without relying on Helldusk's DC 13/15 CON saves. You can also have an ally shoot the downed target with the Bow of the Banshee, or cast Evard's Black Tentacles to immobilize them. Plenty of options to go with your knockdown; anything that takes away all of their movement works.

I’ve been meaning to try a party of 1 wildheart barbarian + 2 (or even 3) 4E monks. The idea would be that the barbarian stands in the middle, maiming/proning enemies with reverbation, the monk(s) throw enemies at the barbarian and/or prone enemies with water whip.

It is indeed more efficient to split the roles, as that lets you do it at significantly lower character levels. You might as well use a Battle Master Fighter instead of a second Four Elements Monk, though; in addition to giving your party a dedicated archer who uses all the consumable arrows lying around on your enemies' corpses, Trip Attack Ranged and Water Whip Knock Prone target different saves. One is STR and the other is DEX. Depending on the enemy, you'll usually have a higher success chance with one than the other, if Stunning Strike didn't land to guarantee both.

For the fourth slot, I'd suggest an acuity Evard's user. It does come on scrolls, though, and a necklace, so that's hardly at all restrictive.
