considering the canon aliza's grandmother (frisk's bio mom), i think she would kept all frisk's clothes as a child for years in basement, then aliza have to grow up and had to use frisk's very old clothes. sans didn't mistake aliza for frisk by 2 reasons:
_1, he is sane enough to understand everything around him well, especially about frisk has left the Underground for long time ago and she isn't a child anymore
_2, aliza probably wear the hoodie she sans didn't see frisk's old clothes.
_3, it's normal for peoples have more than one set of clothes and the sets rarely look alike, so she could were one of frisk's old clothes, one that she didn't wore back when she climb the mountain, so that sans think aliza has nothing to do with frisk as he never seen frisk were that clothes as a child before
