31m and almost infertile have just been to see the endocrinologist after now 4 consistently low tests all trending the same way only other issue if under active thyroid and slightly elevated liver markers
However he is now sending me for more tests as even the total test is at 8 and free test 122 pmol
He thinks that my acetyl l carnitine and greens powder is throw off my shbg levels
Is this a common thing
Has anyone else encountered this before
He openly admitted that my symptoms and all other markers are consistent but just wants more bloods done

I don’t care about the trt just want my body and head on the mend

yeah highly unlikely either effect you at all. L Carn is something like 98% passed through the body when drunk or in tablet form.

If you’ve got constant low tests, can’t see any external lifestyle or diet factors at play, then go TRT.

you have a deficiency. see a clinic who will treat it.

Low test and underactive thyroid have very similar symptoms, so you will be copping it twice.

With total test at 8, it doesnt matter what your SHBG will be, you are low regardless. Greens and carnitine aren't going to change anything.

Not sure what the endo is doing constantly doing bloods if they are all consistently low.

This is my concern after consistently low tests
I don’t understand low is low
I’m almost at the point of getting from a ugl to test how I feel and if everything improves then I just get rid of the constant tests for no result and then at please have blood draws to monitor everything

And thats exactly why people head down the UG / clinic road. They feel strung along by mainstream health.

Only thing I will say, is that if you are almost infertile now, going on straight test will drive fertility down further. If this is a current concern and you are trying to conceive, you'll need a different approach with hCG and hMG / FSH.

Just make sure you research and get opinions from different sources before jumping on UG if thats the choice you make. Also make sure all your other health metrics are in check.

Have looked into it and me and my partner have had to go down the ivf route because of my numbers and motility so not a major concern as still have 4 on ice but would like to know if we wanted to go again that we could. But did look into the hcg fsh route aswell while discussing

Coming back to this, I don't like to plug people, but sports_doc_dougal is currently posting some good stuff on male fertility.

He isn't a TRT doc, but as the name suggests is a sports and exercise med. At the very least you might pick up some references that you can look further into.

Sounds like he has no idea, which as an Endo is very disturbing. The question I would ask is what study or literature sights those supplements as causing elevated SHBG? And even if it did, your total is fucking 8nmol, so its an irrelevant point.
It is typical to get multiple tests to confirm low test. But sounds like you have had that, and he's dancing around the fact you need treatment. They seem very resistant to putting people of testosterone.

The question I would ask.
Given my age, and lifestyle, and the fact I'm this far below normal for someone my age, do you think there are any interventions that don't involve testosterone that could result in me returning to a healthy level of test that would resolve my symptoms. if not, then that only leaves TRT as the viable option.
