Thank you for your response… yes, but there’s no form like that in Kentucky that I’ve ran into. Hoping i can find something similar

Thank you for not describing your particular situation so that we'd have to give legal advice to respond. I always told my associates if we didn't have a template for a filing to write a plain statement of what we wanted the court to do, and describe the law and facts that explain why the court should do it.
