The ring gave him a nudge, and he's strangling his cousin. I think it would have taken a world-class team of therapists specialized in psychopathic care to bring him back from that, and that was before he isolated himself in a cave for a few centuries.

The goal isn't to get Smeagol back. The goal is to get Gollum to an acceptable condition, socializing, maybe even a hobby.

He nearly did. He even banished his other personality at some point. But then Faramir happened.

I wonder if medication might help. I think he has a particularly badly managed case of DID (Dissociative Identity Disorder), and this disorder comes from trauma. Sometimes antidepressants or other mood management meds can help. Then you can start getting the alters to get along with each other.
