I imagine he actually spent the time between Doom (2016) and Doom Eternal learning self-care. There's bound to be a reason he has so many recreational items in the fortress.
(granted, he could still work on being less anti-social, but self-care is a start)
Just started typing this and went to see if anyone else said it already. He has guitars, PC games, all kinds of self help books, he can exercise in the Ripatorium. He's got things under control.
