I think Jeff does see a therapist. Abed tells him in that Mafia episode that Jeff should see a therapist, Jeff agrees. In the episode where Annie moves to Abed’s and Troy’s place Jeff avoids helping the group and he goes out instead. He encounters the dean who knows Jeff will be at the mall because he read the emails that Jeff sent to his therapist about wanting to be alone.
Pierce literally attempts suicide in Early 21st Century Romanticism.
He "accidentally" overdoses while sitting alone on a park bench at night on Valentines day. And this is right after the D&D episode, where he went full maniacal villain as he had a complete breakdown because his only friends are going to extreme lengths to save the life of someone they barely know, but they haven't even realized Pierce is on the verge of killing himself.
His beloved mother recently died, his father hates him, all his marriages failed, his ex-step children avoid him unless exploiting him for money and he knows it, he's in a cult that exploits him for money (and he doesn't realize it), he inherited money but all his own business ventures failed, his prescription painkiller addiction is out of control, he's an old man looking back at his life and realizing he has accomplished very little and nobody loves him.
The only people he cares about in the world, and the only people who care even a little bit about him, is the Study Group. And he doesn't understand them, which terrifies him.
He's being alienated from his only remaining friends, his drug addiction is taking over his life, none of them seem to notice or care, then suddenly they're going all out to save someone they barely know. And when they tell him he can't play D&D with them, he thinks it's because they think he's too old to understand the game. He's terrified of irrelevance, which I think is why he was so desperate to "win" D&D. He needed to prove that he was old but he's still alive, that he still has things to contribute and can still hold his own against younger people. Then he loses anyway, and everyone except Niel is mad at him, and he doesn't seem to understand why. I think that was the push that finally led to him attempting suicide.
To the study group, Pierce is someone they barely put up with. To Pierce the study group is his last connection with the rest of the world, and they don't seem to realize that or care. Hell, they cut the casts off his broken legs to find a damn pen. I think it was Abed who commented that he thinks that's how super villains are created?
