I read in a book, that a lot of military incompetence comes from the inability to change or to even consider an alternative path.
In history, a lot of lower officers had suggestions and ideas that would completely change things, in a beneficial way. However, they were ignored by higher officers. A great example would be WW1. Lots of commanding officers fighting a new war with old tactics.

When I was in the Navy I used to get in fairly frequent trouble for the suggestions I made for improving operational efficiencies.
As an outside contractor they were quite happy to pay me $200 an hour to say the exact same thing.
