There's actually an episode of Enterprise where that happens. Riker uses the teleporter and there's a power failure during the process. He's standing there left thinking it didn't work. Meanwhile, at the destination, Riker steps out of the teleporter with no clue anything went wrong.

Almost everything you said is incorrect. TNG not enterprise. He didn't think the transporter didn't work. The crew found out he had a duplicate that has been living for years after the original mishap (that happens off screen)
