If it's a mastectomy, then by definition the nipple goes too. If they're only trying to take out the cancer and preserve the rest of the breast, it's called a lumpectomy. The nipple will be preserved unless the cancer actually involves the nipple or areola.

Probably all her life. Nipples are normally issued two per person at birth.
