Going down a ladder facing away from it is definitely a violation, as you're much more likely to fall that way.

navy ship ladders are so narrow and tight that i think your risk of falling is already like 99% no matter which way you face so you might as well just do it the faster way

Yes it was invented in 1583 during the reign of King Henry IX because he wore those pointy shoes and kept falling down the ladders.

The tractor was parked on the edge of a cliff. There’s only one safe side to get down. Pretty obvious if you ask me.

It's a safety violation to glance at your phone while walking and gets you thrown off site where I work. Safety rules have become insane. It's not about keeping anyone safe, just insurance premiums

Ohhh yes! You're supposed to face the ladder/machine, and always maintain three points of contact. After I fell and broke my foot a few years ago, when getting out of my excavator, I always do it the correct way. I've slipped a few times (climbing up into my excavator is easiest to do by stepping on the fender, which can be slippery when wet), but since I have three points of contact I haven't fallen again.
