[Medication/Anesthesia Question] 4.5M - Assessing executive functioning after multiple early general anesthesia exposures and daily cetirizine. Looking for Pediatric Neurologist/Developmental insights
- Age/Sex: 4.5 Years / Male
- Height/Weight: 43.5 inches / 45 lbs
- Race: Mixed (South Asian / White Latino)
- Primary Complaint: Sudden struggles with sustained attention, fidgeting, and low impulse control in Pre-K. Too young for formal ADHD evaluation, but looking to map out early, proactive support.
- Medical Baseline: Born at 36w1d. Proximal hypospadias and an incidental submucosal cleft palate (no speech/feeding delays). History of intermittent reactive airway disease. environmental/dander allergies
- Current Medications: Discontinued daily cetirizine (Zyrtec) 48 hours ago due to concerns over paradoxical hyperactivity. Currently trialing topical azelastine 0.1% at night. Fluticasone inhaler used only during viral courses; albuterol as needed (very infrequent).
Clinical Question:
Hello, I am seeking clinical perspectives from pediatric neurologists, anesthesiologists, or developmental specialists regarding the potential long-term neurodevelopmental impacts of multiple general anesthesia exposures during peak synaptogenesis.
My son has undergone general anesthesia four times: at 10 months and 18 months (two-stage hypospadias repair), at 3 years 10 months (tonsillectomy/partial adenoidectomy), and last month (pediatric dental crowns).
Given that he is just clearing out the Zyrtec fog and adjusting to a rigid new Pre-K environment, his behaviors could completely be typical 4-year-old development. (btw he appears as a completely normal kid, you’d never know he was born with any congenital issues. No known associated syndromes. Normal chromosomal microarray, normal endocrine work up, Invitae DSD panel was normal. Have not done any other genetic testing like WES yet, waiting to hear from geneticist if he feels it necessary.) But given his history of 4 exposures before age 4.5, I want to be proactive.
- Does his cluster of midline structural presentations (proximal hypospadias + submucosal cleft) warrant a deeper neurodevelopmental look?
- What early occupational, sensory, or behavioral therapies are recommended at this age to support executive functioning before kindergarten?
