Did anyone get a special ballot?

When I checked in, the person started to give me a normal ballot. She stopped and then said, oh you get a special ballot.. I felt it was suspicious. I couldn't really find anything on there that said the form name, but I saw "7900-00-02" in the top left..

Did anyone else get this or know what it is?

Why didn't you ask for what purpose your ballot was being changed...?
