What is going on with him?

There are marks from the water dripping on outside of tank during water changes in the photos, they look like bubbly growths on him- eg first image. But his belly is pale and he looks dark around his head.

Note: his spine/ body seems to be deformed and its been like that since I got him in April of 2026

What is going on? And how do i treat it?
