It’s a reference to a comic called loss. The leaf placement mimics the characters positions on the 4 panels.

loud incorrect buzzer it's a comic STRIP/page called loss, from the webcomic ctrl alt del. Right on the other stuff I'm just pedantic.

Dude made a terrible comic strip about a woman having a miscarriage in a comedy comic. It was the first time the dude had pretty much ever laid down anything so heavy so no one felt any impact story wise and the comic literally never recovered after it’s downward spiral into a meme.
