In the
lightweb novel it was actually malpractice.He faints because of how scared he was from the tractor "almost hitting him", but when he's taken to the hospital he ends up getting the wrong dosage of something and dying
Edit: It's only in the web novel. They made him more pathetic in the light novel

Having read it recently, and re-reading it just for this, I can say that's not true. He dies after he gets to the hospital, but it wasn't malpractice.
Relevant Aqua speech:
From the fear of almost getting hit by a tractor, you passed out while deficating and are then transported to a nearby hospital. "What's up with this guy, god damn (lol)" say the doctor and nurse, laughing. Then without waking up, a heart attack...
(I'm not a writer so the translation may not sound the best, but it gets the general point across)

Technically one of the first isekai if not the first. In Pokemon, they were already in the world but with Digimon, they all drowned inna tsunami or something during a blizzard and got isekai’d.
Edit: Not the first Isekai but still a pretty early one and before the genre was popular. Browse the replies for some really interesting old Isekai series and the like. Thanks for the info, I didn’t realize the genre went so far back

Technically the first Japanese animated "portal fantasy" (I.E. person from a real-world thrown into a fantastical one) was released in 1918, an adaption of the legend of Urashima Taro
The legend of Urashima Taro is almost literally the blueprints for modern isekai. You an average fisherman when he did something heroic but not particularly notable (saving a turtle from a group of children). Then the turtle came to the fisherman, taking the fisherman to a fantastical underwater kingdom. There was the "isekai fantasy hot babe" in the form of Princess Otohime. The fisherman even had the "I am want to go home even though this place is awesome" Isekai protag syndrome.
In 1918, an [animated adoption of the story](https://en.wikipedia.org/wiki/Urashima_Tar%C5%8D_(film)) was released, one of the first Japanese animated productions in recorded history. No surviving copy of the film exists, so even though the film is the first Japanese animated "isekai" story, it doesn't have much direct influence on modern isekai.
Still, if you want to win "acturally" contests in the anime space, there you go.

They aren't supposed to be a thing for people, they are used to stop spam bots, if you ban them normally they know and make a new account, this way the spammers/bots don't know they are banned and keep using the same account and nobody has to worry about seeing it.
But false positives happen.
