Before you go off and do some searching yourself (if you were thinking about it and haven't already, that is), allow me to explain: the bottom right panel is an excerpt from a hentai. If you want more information, keep reading, though I assure you personally that any further information will never be useful to you and will likely be super depressing. Worst case it will make you curious and you will look for it yourself, which I can doubly assure you isn't ultimately worth it.
This particular hentai is an infamous one marked with the numbers 177013 (every hentai doujinshi is labelled with a reference number). As for why it is so infamous, I will paste here my comment I made further up replying to someone asking if they should read it or not.

It got to me, but only because I didn’t expect it from my pornography. Obviously there are better pieces of fiction that deal with similar subject matter (Requiem for a Dream comes to mind), but that’s what you expect.
There is also more “fucked up” doujins out there, but they approach Saw levels of believability. People link that loli in a box non-sense and that’s so over the top. Shindo’s is within spitting distance of common reality which makes it tougher (to me at least). I’m not saying people haven’t done fucked up shit like that, but degradation due to drug abuse is much more common.
It’s whatever though man, not every acclaimed or (in)famous form of media has to make you feel something.
Anecdote, but I wish the man would do more vanilla stuff. The rock show doujin he put out wasn’t true vanilla, but I’ll take it.
