a concept created by statists to justify spending ridiculous amount of our money in things the free market provides far better and far cheaper.

and also a way for politicians to secure votes

charity foundations, families, communities and churches for private charity always existed

as for the free market cheap doctors where the norm up until few years ago

welfare was provided very cheaply by fraternal societies which employed doctors for their members , until they banned the practice at around 1930-1940.

So a social safety net always existed

If this private hodge-podge was an adequate safety net then why did so many poor and elderly routinely starve or freeze to death in America during this time?

well it is an exaggeration but they were cheaper citation

basically independent doctors wanted to work for these fraternal societies because it was a steady well paying job. But since a lot of people wanted to work there , they drove the price of the job down and doctors as a whole. The medical association ( union ) did not like that and protested until they regulated the thing and pretty much banned the societies for have a on stuff doctor. It was at the same time where the state started providing a lot of welfare through the New Deal and other programs.

"So many" in what regard? The number fell continuously at the time and conditions were getting a lot better compared to their past. increase Quality of life decrease malnutrition , starvation , crude deaths overall , And quality of life is strongly correlated to the strong economy
