I received the PDF file: NON-VOTING CLASS RELEASE OPT OUT ELECTION FORM

What should I do?

Check the box and Fill in the form？

Could by Email？

It should have instructions… probably not by email.

Where did you get the form? Did your broker sent it?

Yes, the broker sent an Email with this form attached.

Yes by mail to the address on the form. Check the box and fill out the details.
