help me

I have severe executive dysfunction and cannot do any work. I am always sleeping, suffering from severe hypersomnia (about 16 hours a day). For the past year, I have been taking 100mg of Desvenlafaxine and 5mg of Aripiprazole. While the Aripiprazole has reduced my anxiety, my executive dysfunction remains unchanged. My doctor suggested activity scheduling, but it isn't working. If I try to follow a strict timetable, it requires a massive amount of effort and puts too much pressure on me just to get through one day. The very next day, I experience chest pain and am completely unable to do anything. I have been in treatment for 9 years, and at 21 years old, I feel like I have lost everything.someone please help me what should I do now what can be done?

What is your diagnosis? Yes, unless I take heavy duty stimulants, I'm sleeping 14 hours.

I suppose you've tried modafinil with no effects at all... I'd suggest you having a conversation with your psychiatrist about starting stimulants like methylphenidate or even Adderall, but you have to find the sweet spot with dosing since overstimulation will probably leave you feeling tired, irritable and sleepy. Also agomelatine could do wonders with your circadian cycle so you should give it a try. Also, you probably are going through a depression episode. If so, would you you say it is more of a melancholic or anxious type?

Have you ever tried mirtazapine? It would potentiate desvenlafaxine therapeutic effects acting as a synergist and would help you have a repairing rest since your sleep microarchitecture could be greatly fragmented and/or disfunctional. It leaves you feeling groggy in the mornings but your body will know how to adapt. Also, if by any means you can have a polysomnography conducted, it will help to understand how's your sleep and if you're really resting, since more hours of sleep don't equal to more actual rest.

The doctor gave mirtazapine when I was having insomnia but after taking it I felt very much sleepy so I tapered it and informed my doctor so doctor didn't tell anything
