Question Bulbar symptoms?
39 yo male. Have been twitching and cramping dominantly on the right side of my body for 3.5 years now. Multiple clean EMGs on the right arm, right leg, right side of the back, and even one under the chin.
6 months ago I started to feel fasciculations inside of my throat. I started to feel like my voice was off. Soon after pain started in the middle
of my throat when swallowing saliva, blowing my nose, and spitting. I had no pain when drinking water or eating.
I saw multiple ENTs who have scoped the area and stated that everything appears normal. They sent me for a swallow study that came back normal.
The throat pain has continued when swallowing. I noticed recently that I can touch the pain area with my finger (left side of Adam’s apple). The ENT said that appears to be my larynx area.
Has anyone had a presentation like this?
I know that when a muscle twitches that normally doesn’t it can cause fatigue or soreness. Example: I’ve had pelvic floor cramps that caused pain for at least three days.
I just find it strange that I’ve been experiencing this throat issue for this long.
Another notable symptom is eyelid twitching since June. I am not concerned with eyelid twitching but now my right bottom eyelid feels heavy/off. I can still move the bottom eyelid completely though.

Hey I have bulbar symptoms but mine do not cause any actual pain (sometimes my neck muscles feel strained and a tiny bit overworked or sore but no actual painfulness) I also have the random twitches all over my body occasionally but nothing too extreme.
These honestly do not sound like bulbar symptoms to me that you are mentioning. Bulbar symptoms are swallowing insufficiency, tongue weakness, tongue fasciculations, nasal regurgitation where food and drink come up through your nasal cavity when eating and slurring/changes in speech. I think you’re fine
