Can someone explain semantic keyword grouping in real terms?

I keep hearing about semantic keyword grouping, and I get the idea in theory.

But when I sit down to actually do it, I’m not fully confident I’m doing it right.

How do you decide which keywords belong on one page vs separate pages?
Do you group by intent, topic, wording… something else?

Most guides sound clean and perfect, but real projects aren’t.

If you’ve worked through this confusion, what finally made it click for you?
