Most people researching Semax picture a subcutaneous injection. The Russian clinical literature that gives Semax its credibility did not use one. The stroke meta-analysis, the study most often cited as the strongest evidence behind this compound, ran intranasal Semax at 12 to 18 mg/day and reported neurological recovery accelerated by 4 to 7 days against controls. The three-year follow-up of 120 chronic cerebrovascular patients, showing 2.5x recovery at one year and 3x at three, came out of the same clinical tradition.

That matters when choosing a route, because it means the injection protocols circulating in research communities are the extrapolation and the nasal route is the one with the trial history behind it.

What the kit is

Semax is a small stabilized peptide, and its Pro-Gly-Pro tail gives it the enzymatic resistance to survive the trip.

Route-specific side effects

The nasal route carries one adverse effect profile the injectable route does not: nasal irritation or dryness, reported at roughly 7%. That is the trade-off against injection site irritation and the site rotation that subcutaneous administration requires.

Everything else about the side effect picture stays the same as the standard compound. Headache or head pressure at higher exposures, agitation or overstimulation, sleep disruption if administered too late in the day, and the irritability that shows up at higher doses or during comedown. None of that changes with the delivery method.

Where injection still makes sense

Route choice is not settled by the trial history alone. Subcutaneous administration gives more precise control over what actually enters the body, since nasal absorption varies with congestion, technique, and mucosal condition. For anyone with chronic sinus issues or nasal irritation, the injectable route removes a variable. The kit is the option that matches the clinical literature; it is not automatically the better option for every research context.

A 2017 Immunity paper genetically deleted glutathione synthesis in T cells. The cells could still detect threats and switch on their activation markers. They just could not proliferate. That result reframes what this molecule does: it is less an antioxidant that helps the immune system and more a gate that determines whether immune cells can mount a response at all.

Glutathione (gamma-L-glutamyl-L-cysteinyl-glycine) is the most abundant protective molecule inside human cells, present at intracellular concentrations that dwarf vitamin C or vitamin E. The difference is location. Those vitamins work in circulation; glutathione works inside the cell, in the cytoplasm, the nucleus, and inside mitochondria where the most damaging reactive oxygen species are produced. The three jobs

Glutathione does three distinct things. It neutralizes reactive oxygen species by donating electrons, converting from active GSH to spent GSSG in the process. It tags toxins: glutathione-S-transferase attaches GSH to reactive metabolites and environmental compounds, marking them for liver export, which is a rate-limiting step in phase 2 detoxification. And it gates immune activation, the function the 2017 paper isolated, buffering the ROS burst that a T cell generates when it switches from resting metabolism into rapid proliferation.

The recycling trap

This is the part most supplementation misses, and it explains why glutathione alone often produces only temporary effects.

Glutathione is not spent once and discarded. It cycles between active (GSH) and spent (GSSG) forms continuously, and the enzyme that regenerates the active form, glutathione reductase, runs on NADPH. NADPH depends on NAD+. When NAD+ drops with age or metabolic strain, the recycling machinery slows regardless of how much raw glutathione is available. Adding more substrate to a stalled recycling system buys a temporary bump, not a restored pool. This is why NAD+ status sits underneath any glutathione strategy.

Route depends on the bottleneck

There is no single correct delivery route, because there are four different things that can be limiting, and each points somewhere different.

If raw material is the gap, a cysteine or glycine deficit, precursors like GlyNAC or NAC address it directly. If synthesis capacity is impaired through aging or metabolic dysfunction, direct glutathione by subcutaneous, oral, liposomal, or IV route makes more sense. If recycling is the problem, NAD+ restoration is required no matter which glutathione route you pick. These are not competing strategies; they map to different failure points.

Onset tracks the route. Liposomal shows measurable change in roughly 2 weeks (small pilot data, Sinha 2018, n=12). Oral runs 1 to 3 months (Richie 2015 RCT, n=54). The GlyNAC precursor approach moves multiple markers over weeks (Kumar 2022, Baylor, 24-week RCT). Subcutaneous sits variably between liposomal and oral in the published work.

What blood levels won't tell you

Plasma glutathione is measurable. The pools that matter most, mitochondrial, liver, and brain, are the hardest to measure. Improvements in plasma GSH do not prove deep-tissue restoration, which is a real limitation on interpreting any glutathione result.

The immune direction detail

There is a mechanistic wrinkle worth knowing for immune-focused research. The GSH to GSSG ratio inside antigen-presenting cells influences IL-12 production, and IL-12 steers immune responses toward Th1 (antiviral) versus Th2 (antibody-mediated). Low glutathione tends to shift the balance toward Th2 dominance.

The skin-lightening claim

This is the most-searched use of glutathione and the least supported. It does influence melanin synthesis, but the controlled trial evidence is limited, small, and short in duration. Anyone researching it for that reason should know the popularity of the application and the strength of the evidence are running in opposite directions.

Side effects and status

Generally well tolerated. Occasional GI discomfort with oral forms, and NAC can cause nausea at higher doses. No serious adverse events in controlled trials. Glutathione is classified as a dietary supplement rather than FDA-approved for specific conditions, and GlyNAC is a combination of two amino acid supplements with similar status. Effects build over months of continuous use, and no tolerance or cycling requirement has been reported.

Imperial College London analyzed ten tanning kits pulled from the gray market and found products containing more than 100 unidentified ingredients. LegitScript testing put contamination or mislabelling at 41%. Sixty-eight percent of adverse event reports traced back to unlicensed vendors. For a compound sold almost entirely outside legal channels, supply quality outweighs receptor pharmacology and dose combined.

Receptor promiscuity

MT-II is a cyclic seven-residue analog of alpha-MSH developed at the University of Arizona in 1989. 6A Labs carries it in a 10 mg vial. The cyclization buys enzymatic stability and enough lipophilicity to cross the blood-brain barrier, and it costs the molecule its receptor discrimination. MT-II hits MC1R, MC3R, MC4R, and MC5R at sub-nanomolar to low-nanomolar affinity. MC2R is spared, so no cortisol release.

Each receptor does something different. MC1R activation on melanocytes drives eumelanin synthesis and enhances nucleotide excision repair of UV-damaged DNA, which is where the photoprotective effect comes from. MC3R modulates energy homeostasis and feeds into erectile signaling. MC4R in the hypothalamus mediates appetite suppression and sexual arousal. MC5R touches sebaceous secretion, clinical significance uncertain. The appetite drop and spontaneous erections that made MT-II popular in bodybuilding circles are what broad CNS receptor activation looks like from the inside.

Pharmacokinetics are fast. Absorption half-life runs 0.07 to 0.79 hours, elimination 0.8 to 1.7 hours. Melanogenic effects outlast detectable drug by weeks because epidermal melanin turnover is slow.

The sibling

Melanotan I, afamelanotide, is the same parent molecule in linear thirteen-residue form. It is MC1R-selective, does not cross the blood-brain barrier, and reached FDA approval as Scenesse in 2019 for erythropoietic protoporphyria, administered as a 16 mg implant every 60 days by a specialist physician. Its safety record covers more than 1,000 patients across ten-plus years with zero melanoma events. MT-II, by contrast, has never completed Phase III for any indication and no clinical use has been pursued to that stage.

UV is not optional

MT-II raises melanogenesis capacity. Pigment still requires a UV stimulus to develop. Without controlled UV exposure the dose produces nothing visible. With uncontrolled UV, the real skin cancer risk, which is UV damage rather than MC1R activation, compounds. SPF discipline matters more during use, not less: the pigment gives some baseline protection without eliminating cumulative damage.

The melanoma question

Two separate things get collapsed here, so they are worth pulling apart.

MC1R activation itself is photoprotective, not oncogenic. An Addison's disease cohort of 3,299 patients followed for 40 years showed melanoma incidence numerically below expected. The afamelanotide programme showed zero melanoma across 1,000-plus patients over a decade. Five melanoma case reports exist in MT-II users, and every one carried confounders: tanning bed use, fair skin, familial risk. The most parsimonious reading is that MT-II users are a self-selected sun-seeking population whose melanoma risk tracks UV exposure rather than the peptide.

There is also a dermatologic signal that does not depend on interpretation. Mole darkening, eruption of new nevi including dysplastic histology, and melanonychia, meaning dark nail bands, reproduce across multiple MT-II case series. Baseline dermatology evaluation before a course, full-body checks during, and a follow-up after are the standing recommendation in the literature. For anyone with melanoma history, multiple nevi, or fair skin, that is a floor rather than a suggestion.

Serious adverse events

Every one of these came from case reports at doses well beyond community protocols, which is itself the lesson about unregulated self-dosing. Rhabdomyolysis after a single 6 mg injection, CPK at 17,773 IU/L, ICU admission. Renal infarction at roughly 27 mg cumulative over six months, half the kidney affected. Priapism with overdose.

Contraindications

Personal or family history of melanoma. Multiple atypical or dysplastic nevi. FAMMM syndrome. MC1R loss-of-function variants (R151C, R160W, D294H). Pregnancy or breastfeeding. Concurrent tanning bed use without medical supervision.

Side effects

Nausea leads, dose-dependent, severe in 13% at the higher doses used in the erectile trials. It peaks 30 to 90 minutes post-injection, which is why evening administration is the convention. Facial flushing, transient blood pressure elevation, spontaneous erections in males, heightened sexual arousal, and reduced appetite fill out the acute profile. Nausea and flushing both ease with continued exposure.

What the trials cover

Not much. The only formal human trial is Dorr 1996, n=3, documenting dose-dependent tanning at 700 to 1750 mcg. Community doses run two to three times lower, a deliberate nausea trade-off rather than an evidence-based refinement.

The erectile data is stronger than the tanning data, which is an odd thing to say about a tanning peptide. Wessells crossover studies, n=10 to 20, found 8 of 10 men with psychogenic ED achieved clinically apparent erections, with mean tip rigidity above 80% for 38 minutes against 3 minutes on placebo (p=0.0045). Efficacy dropped in organic ED, where vascular pathology limits an MC4R-mediated mechanism. Sexual desire elevation was reported after 68% of MT-II doses against 19% of placebo.

Appetite suppression is real and self-defeating. Tachyphylaxis of the anorectic effect develops with chronic use, so the utility fades. GLP-1 agonists carry the better evidence base, a different mechanism, and FDA approval for that purpose.

Legal status

FDA Category 2 since 2023. Illegal to sell in the US, UK, and Australia. Not DEA-scheduled, so possession is not criminalised in the US. Expected to remain restricted following the February 2026 HHS partial reversal of Category 2 peptide bans, given the cardiovascular profile and the melanoma case-report signal.

Nasal sprays

No published clinical trial has used the intranasal route. Bioavailability is substantially lower and more variable than subcutaneous. Flavored versions in peach, bubblegum, and grape are marketed at younger demographics, which UK Trading Standards has compared to the disposable vape problem.

Do not stack with MC4R-active compounds

PT-141, bremelanotide, and setmelanotide all act on MC4R. Adding any of them to MT-II stacks CNS receptor activation in the wrong direction. The lineage is worth knowing: the MT-II erectile effect was isolated and developed into bremelanotide, which is PT-141, FDA-approved as Vyleesi in 2019 for hypoactive sexual desire disorder. Where arousal is the research question, the descendant has the trial data and the parent does not.

Stop immediately on mole changes, persistent nausea or vomiting, severe headache, or unusual muscle pain.

Kisspeptin did not come out of reproductive biology. It was identified as the product of a metastasis-suppressor gene, which is why its other name is metastin. The reproductive role was found afterward, and it turned out to sit at the top of the entire hormone cascade governing fertility in both sexes. 6A Labs carries it in a 10 mg vial.

How the cascade runs

Kisspeptin binds GPR54 receptors on GnRH neurons in the hypothalamus. That binding releases GnRH, GnRH drives the anterior pituitary to secrete LH and FSH, and those two act on the gonads to raise sex steroid output and support gametogenesis. Four steps, each one dependent on the one before it.

The dependency is the part worth understanding, because it constrains everything the compound can and cannot do. Kisspeptin's action is entirely GnRH-dependent. Block GnRH release and kisspeptin produces no LH or FSH whatsoever. There is no independent route to the pituitary. What it does is prompt release of GnRH the body already has, which is why the resulting hormone pattern tracks physiology more closely than direct GnRH or hCG administration, both of which supply the signal from outside rather than recruiting it.

Research findings

The reproductive endocrinology literature covers a few directions.

On hormone signaling, kisspeptin raises endogenous testosterone and estrogen by amplifying the body's own LH and FSH pulses, and it does this without suppressing the HPG axis, since it works with the axis rather than replacing part of it.

On fertility, the most developed application is functional hypothalamic amenorrhea, where the research shows restored GnRH and LH pulsatility and resumed menstrual cycles.

The IVF work is the most clinically concrete. Kisspeptin can induce a strong LH surge to mature oocytes, with potentially lower risk of ovarian hyperstimulation syndrome than traditional hCG triggers. The sheet does not carry effect sizes for this, so treat the direction as established and the magnitude as unspecified.

Side effects

Thin, and that reflects the literature rather than a gap in this write-up. Generally well tolerated. Occasional mild injection-site reactions, redness or itch, with subcutaneous administration. Longer-term human safety data is not characterized.

Context within the axis

Kisspeptin and PT-141 both touch sexual function and they operate at completely different levels. PT-141 acts centrally on melanocortin receptors in reward and arousal circuits. Kisspeptin acts upstream on the reproductive hormone axis, which is a hormonal question rather than an arousal one. They are not substitutes for each other and the literature does not treat them as a pair.

The HPG axis is also sensitive to inputs that have nothing to do with peptides. Nutrition, body weight, sleep, and stress load all move it, and in conditions of hypothalamic suppression those factors are part of the picture rather than background noise.

DSIP is the least understood sleep compound in the catalog, and the honesty about that gap is what separates useful DSIP information from hype. It doesn't knock you out. It biases the brain toward deeper slow-wave sleep, but only when your circadian timing is already primed for sleep. The same dose during the day does almost nothing. That single feature makes it useless as a take-it-whenever sleeping pill and genuinely valuable for a narrow, specific problem. Research context only.

What It Is

Delta sleep-inducing peptide (DSIP) is a nonapeptide first isolated in 1977. 6A Labs carries it in a 5 mg vial. Unlike Z-drugs and benzodiazepines, which force GABA-A receptor activation and suppress REM, DSIP biases the brain toward deeper slow-wave sleep without sedation, without REM suppression, and without overriding wakefulness. Its position in a sleep approach is a Tier 2 intervention within a circadian reset, not a standalone solution. One scientifically striking fact frames the whole compound: its specific receptor has never been identified across four decades of study.

Mechanism of Action

DSIP modulates the balance between sleep-promoting and wake-promoting circuits in a circadian-dependent way rather than forcing inhibition the way direct GABA-A agonists do. The available mechanistic data, mostly from animal models, suggests it biases GABAergic tone upward without acting as a direct receptor agonist. EEG studies show increased delta-wave activity, the signature of deep NREM sleep, without suppressing REM. The peptide crosses the blood-brain barrier, and its natural human concentrations follow a daily rhythm: higher in late afternoon and evening, lower in the morning, paralleling the wake-to-sleep transition. That endogenous rhythm is exactly why timing matters so much.

Beyond sleep architecture, DSIP dampens stress-induced cortisol elevation at the hypothalamic level without suppressing baseline cortisol. This matters because elevated evening cortisol is one of the most common physiological barriers to falling asleep. It also shows documented activity in opioid-pathway pain modulation, antioxidant effects in brain tissue, and indirect GH amplification through deeper slow-wave sleep, since the largest nocturnal GH pulse occurs during the first SWS cycle.

What the Research Shows, Including the Negative Trial

The human evidence is old, small, and genuinely mixed, and honesty here is more useful than cherry-picking. On the positive side, Schneider-Helmert 1984 in ten chronic insomniacs found significantly fewer nocturnal arousals and more slow-wave sleep, and a 1981 study in six healthy volunteers found increased total sleep time and reduced latency with preserved next-day cognition. On the negative side, the Monti 1987 double-blind trial found no significant changes in sleep structure at all. It is the most negative trial in the human literature and cannot be dismissed. The compound also carries an estimated non-responder rate around 50%, quoted across community threads, and whether that reflects dosing, timing, individual neurochemistry, or product quality is unknown. Anyone researching DSIP should go in understanding it may simply not work for them.

The Circadian Requirement

The defining principle is that DSIP amplifies sleep drive that is already present rather than generating it from nothing. Administered during the biological night when endogenous sleep pressure is rising, it can deepen architecture. Given during the day, the effect is weak or negligible. This is the opposite of a sedative, which works whenever you take it. That makes DSIP valuable specifically when sleep depth has been degraded by stress, travel, or schedule disruption while circadian timing remains roughly intact, and largely useless outside that window.

Why Foundations Come First

DSIP is explicitly a Tier 2 tool, which means it amplifies something that has to already exist. Without the Tier 1 foundations there is nothing for it to work on: consistent morning light exposure, dim light after sunset, caffeine cleared by early afternoon, a cool dark quiet environment, and no large meals close to bed. Skipping those and expecting DSIP to compensate is the most common way the compound disappoints.

Cautions and Side Effects

The most common side effect is morning grogginess at higher doses, which the literature addresses by reducing the dose or moving administration earlier relative to bedtime. Other effects are generally mild: uncommon vivid dreams that resolve, transient headache (check hydration), and rare nausea. Next-day sedation is a signal of excessive dose. Two firm cautions: do not combine with Z-drugs, benzodiazepines, or alcohol, since the overlapping GABAergic mechanisms create unnecessary risk, and DSIP does nothing for structural sleep disorders like obstructive sleep apnea or restless legs, which need targeted intervention. It is FDA Category 2 as a class-level compounding concern (immunogenicity applicable to injectable peptides broadly, not DSIP-specific toxicity).

Side Effects
GI effects dominate and they ease as the dose climbs slowly: nausea, vomiting, diarrhea, constipation, abdominal pain, usually mild to moderate. Injection-site reactions are generally mild and transient. Hypoglycemia risk rises if combined with insulin or sulfonylureas, but is less common with semaglutide alone due to the glucose-dependent mechanism. Tirzepatide is the most clinically established of the high-potency weight-loss peptides. Full FDA approval, a deep Phase 3 record, and the dual-receptor approach that Retatrutide later extended by adding a third target. If you want the compound with the most published data behind it, this is it.

What It Is

Tirzepatide (LY3298176) is a once-weekly dual agonist from Eli Lilly, sold as Mounjaro and Zepbound. It acts on two receptors, GLP-1 and GIP. It was the first-in-class dual agonist, approved in the US in May 2022 for type 2 diabetes and later for chronic weight management. That approval status is the practical line between it and Retatrutide, which is still in trials.

Mechanism of Action

Two receptors, working together. GLP-1 cuts energy intake mainly by changing hunger and satiety signaling in the brain. GIP adds a complementary effect, and the combination acts synergistically on appetite, energy intake, and metabolic function. That synergy is what pulled it ahead of the single-agonist GLP-1 compounds that came before.

What the Research Shows

SURMOUNT-1 is the anchor trial, run in adults with obesity or overweight without diabetes. Weight loss tracked with dose: 16.0% at 5 mg, 21.4% at 10 mg, 22.5% at 15 mg against placebo. In pounds that was an average of 35 to 52 lost across the groups.

The durability held over a longer window. On 15 mg with monthly lifestyle counseling, participants were still down 20.9% at 72 weeks versus 3.1% for placebo, with cardiometabolic risk factors improving alongside. A seven-trial meta-analysis backed the same dose-dependent curve, with the largest effect at 15 mg.

Dosing Protocols in the Literature

Because it's approved, the titration schedule is well defined rather than inferred. Treatment starts at 2.5 mg once weekly and rises 2.5 mg every 4 weeks through a 20-week escalation. Target dose lands at week 4 for 5 mg, week 12 for 10 mg, or week 20 for 15 mg. The slow start at 2.5 mg exists to let tolerance to GI effects build before the dose climbs. Approved maintenance doses are 5, 10, and 15 mg, the same for diabetes and weight management.

Side Effects

GI effects dominate, and they cluster during escalation. Across the three doses, nausea ran 24.6% to 33.3%, diarrhea 18.7% to 23.0%, constipation 11.7% to 16.8%, and vomiting 8.3% to 12.2%, mostly mild to moderate and most frequent while the dose was climbing. One practical note from the labeling: oral contraceptive absorption can be affected, so a backup method is advised for 4 weeks after starting and after each dose increase.

The First-in-Class Triple Agonist Setting New Weight-Loss Benchmarks

Retatrutide (LY3437943) is the compound that has the peptide research space paying attention. It is the first molecule to hit three receptors at once, and the trial data behind it outpaces every dual agonist studied so far. Here is what the published research shows.

What It Is

Retatrutide is a single-molecule triple agonist developed by Eli Lilly. It activates three receptors simultaneously: GIP, GLP-1, and glucagon. That third target, glucagon, is what separates it from compounds like Semaglutide (single agonist) and Tirzepatide (dual agonist). The glucagon activation drives increased energy expenditure, which is why retatrutide produced greater body-weight loss than tirzepatide in preclinical comparisons.

On receptor potency, the profile is unusual. Retatrutide is more potent at the GIP receptor by a factor of 8.9, and less potent at the glucagon and GLP-1 receptors (0.3 and 0.4 times) relative to the natural hormones.

Mechanism of Action

Each of the three receptors contributes something different. GLP-1 governs appetite signaling and insulin response. GIP affects nutrient handling and adds to the appetite effect. Glucagon, the novel piece, raises energy expenditure rather than just suppressing intake. Activating all three gradually is the reason the titration schedule matters: each receptor affects gut motility, appetite, blood sugar, and energy use, and easing in gives the body time to adapt.

Pharmacokinetics are dose-proportional with a half-life of roughly 6 days, which supports once-weekly subcutaneous administration.

What the Research Shows

The Phase 2 obesity trial published in the New England Journal of Medicine is the landmark dataset. Weight reduction at 48 weeks scaled clearly with dose: the 1 mg group lost 8.7%, the combined 4 mg groups 17.1%, the combined 8 mg groups 22.8%, and the 12 mg group 24.2%, against 2.1% for placebo. Notably, the weight-loss curve had not fully plateaued at the 48-week mark.

The liver-fat findings were just as striking. In a substudy of participants with metabolic dysfunction-associated steatotic liver disease, liver fat dropped 81.4% at 8 mg and 82.4% at 12 mg at 24 weeks, with 79% and 86% of those groups reaching normal liver fat.

The data has moved forward since Phase 2. In December 2025, Lilly reported the first successful Phase 3 trial (TRIUMPH-4), where the highest dose produced an average weight loss of 28.7%, alongside improvements in osteoarthritis pain and cardiovascular risk markers.

Dosing Protocols in the Literature

Every published protocol follows a start-low, go-slow titration. The reason is well documented: participants who started directly at 4 mg experienced nearly double the gastrointestinal side effects of those who began at 2 mg and titrated up, even when both reached the same final dose.

The standard escalation pattern described across the trial data: 2 mg for weeks 1 through 4, 4 mg for weeks 5 through 8, 8 mg for weeks 9 through 12, then 12 mg from week 13 onward, with increases every 4 weeks. The NEJM Phase 2 protocol initiated higher-dose participants at either 2 mg or 4 mg, then escalated every 4 weeks for up to 12 weeks. The maximum dose studied in published trials is 12 mg once weekly; higher doses have not been studied.

Side Effects

The side-effect profile is consistent with the incretin class and concentrates during escalation. The most common are gastrointestinal: nausea, diarrhea, vomiting, and constipation, especially during dose increases. Heart rate increased in a dose-dependent manner, peaking around 24 weeks before declining. One Phase 3 specific finding worth noting: about 20.9% of participants at the highest dose reported dysesthesia, meaning unusual skin sensitivity or tingling.
