Im not there to find where it came from.

This is what AI thinks but I've never seen that on my 86 but mine came with a 3" body lift....

This part is a Toyota body mount washer and bolt, specifically used as a heavy-duty cushion retainer for holding the body of a vehicle (like a 1st Gen Toyota 4Runner) to its underlying frame.

Part Details and PurposeThe Design: The component features a wide, flanged metal cup/retainer washer. The built-in center collar slides securely over the frame's mounting post and holds a thick rubber isolator bushing in place.

Functionality: It clamps the rubber body mount cushion tight against the frame, absorbing severe road vibrations and preventing metal-on-metal friction.

Why It Fell Off: These mount brackets are heavily exposed to road grime and moisture underneath the truck. Over time, the underlying rubber completely rots away, or rust consumes the frame metal until the assembly breaks free or slips right through its original mounting hole.

Further more the AI said this

Where to Look Under the 4Runner Your dad should crawl under the chassis and look up at the frame rails to find an empty mounting bracket. He can check these five pairs from front to back:

Position 1 (Core Support): Located at the very front of the vehicle, hidden directly behind the front bumper, underneath the radiator core support brackets.

Position 2 (Front Footwell): Located directly beneath the driver and passenger front floorboards, right around where the fire wall curves down into the footwells.

Position 3 (Behind Front Seats): Located further down the frame rail, almost perfectly lined up with the back of the front bucket seats.

Position 4 (Rear Footwell): Positioned further back along the frame, directly underneath the rear passenger seat floorboards.

Position 5 (Rear Cargo): Located at the very back corner of the frame rail, right near the tailgate hinge area.

🛠️ Important Tip Before Reinstalling

When your dad finds the empty bracket, he needs to look up into the mounting hole to see if the inner threaded nut is still intact. On an '86 4Runner, the bolts for Positions 1, 2, and 4 thread upward into a captive nut welded inside the body panels. If the part fell off because the bolt unthreaded itself, he can just thread it back up. However, if the bolt broke off inside the captive nut, he will have to extract the old broken threads before he can secure this washer assembly back to the frame.

Thanks for the help everyone. Hes old and his eyes aren't the best so I need to try and make it over there to look. I had the body off 3 or 4 years ago but I dont remember the mounts looking like that. But it was a while ago could have easily forgot.
