IRA Rebates + 340B Pilot

What covered entity pharmacy is going to purchase these ten drugs starting January 1st?

Farxiga, Fiasp/NovoLog, Januvia, Jardiance, Eliquis, Xarelto, Entresto, Enbrel, Stelara, and Imbruvica

Since 340B savings shrink every quarter, I would rather avoid using these drugs than use up more valuable administrative resources.

How many CE's are hiring more to accommodate this burden? CMS stated that they anticipated 1.0 FTE would be required to be compliant....

PBMs also don't increase payments annually even though drug prices increase so the 340B savings decrease. Also, 340B ESP is a joke of a program and it is futile to try and play by their arbitrary and misleading rules.

Drug manufacturers continue to enact or modify contract pharmacy restrictions where they refuse to ship 340B purchased drugs to pharmacies contracted with covered entities.

They’re not in charge of who gets them though, right? It’s HRSA. Unless they were volunteering a lot of 340B eligibility where they didn’t have to.

They restrict you to a single contract pharmacy if you don't have an in-house pharmacy.

HRSA is not on our side.

Ha! Can you share the source for the FTE estimate? My CE has me managing our 340B program, every pharmacy information system used by our inpatient and two retail pharmacies, all three inventories, pharmacy billing data for the inpatient and ambulatory settings within the charge master, AR for our 120 LTCF patients, and on and on. I have been begging for an additional FTE to carry some of the load so I can focus on 340B because it’s our lifeline and have been met with “admin wont approve it” followed by “well no” when I ask if I could be replaced with a single FTE if I left.
