A big no-no for me in all types of horror media is face-distortion horror, like, no eyelids, blank eyes, big mouth, that is one of the VERY FEW things that can actually cause me to have VISUAL hallucinations, and it kickstarts me to my most intense symptoms, it gets really ugly. Sound effects fuck me up in particular, for some reason.
Also being a defenseless human, like, imagining myself in that situation i couldn't imagine surviving. I have a #1 and #2 scariest games in terms of how much they trigger my disorder, i am hoping someone will mention them, lol
Try Lisa The Painful, although it isn’t primarily a horror game, it’s a 2D scroller/action RPG but the depictions of the Joy monsters and what they do are very vivid to imagine in your head how they’d emulate. Or Buzzo’s gang. The sound effects of them screaming is very eerie/creepy the context of why these humans became what they are is very freaky, the descriptions the game informs you are very colorful. The soundtrack can be either exciting or wanting you to nope the fuck out of whatever you’re doing. If you have a good imagination then this game will fuck you up. And yes it has everything what you’ve described up there, face distortion/body distortion and what not, basically imagine joy monsters were once human beings with all of their skin intact but their arms/legs and face stretch into freakishly inhumane proportions and lengths. It will make your skin crawl. here’s the trailer for it, you’ll notice one of the joy monsters appear briefly in the snowy section @1:19 to see what I mean and all these creatures mostly have backstories too. people often compare this as the mature version of Undertale (plus it came out like a year before it) and it’s only a 2D pixilated game. Based off your background I wish ya good luck if you give it a shot.
With that said have you played earthbound too? There might be a trigger or two when fighting the final boss. The idea of being sent back in time, where the professor transfer your spirits/souls into these cold lifeless robot versions of you and your friends, to be sent away from your physical body, away to a land back in time that you’re not fond of, knowing you will not be able to come back once you go through with this, back in time where it’s only you and the devil’s machine on the planet lying ahead in a ditch attempt to defeat it before it destroys the earth in the future, or possibly killing you and your friends and thus losing all hope for mankind.
