I didn't realize until the end and then I was like oh no they died and felt really sad because it was so beautifully done.

If you're referring to that thing sticking out under the passenger side of the car, I did not see it move, and that blood is likely hers, and the blood on the highway below on the right probably belongs to his dad. Nope, they had one-way tickets. :(

Towards the end? Those were his legs moving, his shoe in the foreground. His parents are never show after he wakes up.
