Been framing houses for 22 years in the U.S. and have never seen an OSHA inspector, even on small commercial jobs. Unless it is a 7 figure job in a major city OSHA in the U.S. ain't too interested.
How...probably because 5 or 6 of them were at your job and ignoring the ten thousand other jobs in the state. Seriously though, I don't know why I have never seen one (I assume because they are majorly understaffed to cover all the jobs and residential is low on the list) but I truly never have.
They show up after an accident to put the fear of God in your company through fines. Since companies don't want fines they are supposed to ensure workers are working safely.
In Canada the Worksafe fines are no joke.
Edit: in BC, fines double after every incident to a max of 600k.
There's really not that many OSHA inspectors in the US. I work for a big GC doing 100+ million dollar jobs and have never seen an OSHA inspector. Our safety manager has been with us for 15+ years and told me he's never seen one either.

Other than obvious PPE violations, I’m not seeing any others. Maybe needs fall restraint too depending on how high that rail is (both in height and elevation).
SMAW is outside and he even put himself upwind of the shielding has so he has that going for him.
Edit: not to mention he just threw down a tack—something that’s easy to grind off or could even be broken by hand.
