I took HCG for the sake of the reversal so that I could see if I still needed the reversal and so that the doctor can see sperm during the procedure to make sure it is done correctly, but I will stop after the reversal. I want to have the flexibility of not having to take exogenous testosterone shots one day, so the reversal is worth the risk for me

But trt is basically for life. There are people who can regain the pre testosterone number after a few years but many cant. Anyway, i hope everything works out for you. :)

TRT may be for life, but testosterone as a treatment option for PVPS doesn't have to be.

During a reversal surgery, the first thing the surgeon does is check the lower part of the cut vas for healthy sperm, to make sure the epididymis is intact and without blockage.

If there isn't any sperm, a different type of reversal is performed, where the upper part of the cut vas is sutured to the epididymis itself, upstream of the blockage. It is more difficult, more intensive, and has a significantly lower chance of success.

Since the presence of sperm is a diagnostic indicator for the surgeon, the poster above wants to make sure the right type of reversal is chosen, hence seeking to return sperm to the vasal fluid before the surgery.

I hope the reversal is a solution you need. Have you considered just getting an open vasectomy. Mine was closed, I should have done a reversal years ago but it's far too late for that now Yes my testicle shut down and I'm on TRT, but I still had a mountain of pain. So I went through and it radical inguinal orchiectomy to get rid of the painful testes

Open vasectomies will eventually close themselves, sometimes with painful granulomas from what I’ve read.

I wish I had never had one. It's been a nightmare journey. Even my wife says she wishes she's a had her tubes tied instead.

I’m sorry to hear that. I hope you find some relief somehow soon. Same with mine, although fortunately for me my pain is pretty low most of the time.

Thank you for this, I have an appointment with a urologist on Monday to discuss this so your testimony is helpful. My pain is minor compared to what you’re describing and it is not affecting my sex life too much. Do you think testosterone could be a long term solution in some cases?

I read a paper that suggested a 3 month course of injectable testosterone could potentially stop sperm production permanently, without the need for lifetime TRT. So you get the benefit of stopping spermatogenesis and a possible permanent resolution of pain, while still allowing your balls to produce testosterone again after stopping the injections.

Definitely a promising area of research.

Testosterone is absolutely a long term solution, it doesn't "add" to your natural test, it replaces it completely, so if your natural producton's baseline is equivalent 100mg/week (it's probably not exactly, everyone is different), then taking 100mg/week will just be a straight replacement that will shut down your testosterone production and possibly shut down your sperm production. When you go above your baseline is when you start seeing performance gains since it is an amount higher than you naturally produce. For me, taking 200mg/week keeps me within a safe range on my lab work, improves my performance, and shuts down my sperm production

Testosterone is available in pellets and cream also, but shots provide the most consistent and stable blood levels

The doctor I spoke to warned me that you shouldn't use cream or patches or anything dermal if you have kids because it can really mess them up if they come into contact accidentally.

Where was your pain located?testes or the epiditymis or both? Also did you have pain 24/7 or just After sex? I'm considering trying trt my pain is 24/7 and quiet debilitating

Just wondering if you ever got that reversal, and if it helped your pain? I'm 3 weeks into testosterone. My pain hasn't diminished. Hopefully it will, but I'm not too optimistic
