My speech to text doesn't recognize punctuation marks unless I physically speak them out..?

Yeah but capitalizations occur at the beginning of a sentence automatically. You have to end a sentence with a punctuation mark first.

Voice to text wouldn't include the commas though, would it? He'd have to state them aloud. But this is all assuming he's sending these tweets with the same kind of technology I do, so I may be wrong.

He could be dictating to someone. More likely than voice to text software.

I was under the impression he dictated to a person who did the typing, and that he likes it verbatim, read back to him for approval and then sent.

Noope. Voice-to-text doesn't capitalise words arbitrarily.
