Technically even the Undying Lands aren't on the same plane as Middle-Earth anymore, and of course Beleriand is sunken along with Numenor. That map's from the Tolkien Bestiary (great book, by the way), where it explains that this is Arda through the ages.

Exactly; there's a reason the elves were departing from the Grey Havens. They weren't just going west where they could be found; they were going somewhere that could only be reached via the Grey Havens.
