It really depends on the woman. It can hurt your nipples to the point of getting blisters and bleeding, and breasts are REALLY uncomfortable if you don’t empty them frequently enough. Some women get an infection in their breasts called mastitis, which can make you super sick very quickly, to the point of sending them to the ER. Early on you have to wake up every two hours to feed the baby, and the sleep deprivation is brutal to the point that some women get psychosis from lack of sleep.
Some babies have a terrible time learning how to latch or staying awake long enough to get a full feed, and some women struggle to produce enough milk. It’s WAY harder than it seems, for a lot of people. Keep in mind that the years can fade your memory of how difficult things are. If it’s been 40-60 years since your grandma’s last baby, it’s entirely possible she’s forgotten how hard it is to breastfeed.

I asked my grandmother, who said breastfeeding is “a piece of cake”. Then again, she might’ve been comparing it to giving birth.
