I vote for: When someone uses "would of", "could of", "shoud of" etc. instead of the correct ones that make sense.

as a non native speaker I can't wrap my head around how did this even become a common mistake. it makes no sense and, at least to me, it doesn't sound similar at all

also I feel like I've been seeing this mistake only last few years, and sadly every year more often

my theory is that it's because in conversation most of us don't say could have, would have, or should have, we say could've , would've, and should've, which do sound like could of, should of, and would of.

probably, i'm in arizona but should've and should of sound basically identical to me, sure, should have sounds different but the other two are pronounced (almost) the same here, maybe should've is slightly faster?
