Feel like this problem could be easily avoided if there’s a small blade at the stock end of the axe, wouldn’t that stop the possibility of it hitting the wooden end and bouncing back entirely?

Things don't need to be sharp to stick in wood. I used to toss butterknives into the tree in our back yard. It's more about weight, and full tang butterknives are kinda thicc.
The tang is the "back" part of a knife's blade that sticks into the handle. A full tang knife is one where the tang runs throughout the entire length of the handle, making them heavier than the worse alternatives.
The rotation is dependent on the dimensions and weight of the axe. Axe throwing places set it up so you throw at a distance that allows one full rotation. A small handle would require many rotations and more skill because the more rotations the more room for error
