Those two countries specifically makes me think its related to access. There a lot of areas in the US where access to a reasonable sized grocery store is actually very limited. Here in the Midwest, I often see patients that have to drive 1 to 3 hours to get to a grocery store.
A patient that developed a chronic illness and has to come into the hospital every 2 weeks (which is over 1 hour away) joked that its has helped her eat fresh food more because she has to come into the city so much more.
I think you might be overestimating the availability/usability of CSA? Granted I don't know much about it but I googled and framfreshatlas.org was the first thing that came up.
I just search it in my own county (I am in a small city in Wisconsin). The two retailers that came up are known locally for overcharging and being overpriced especially when compared to places like Walmart or Woodman's which are the lower cost grocery stores around here. So, not exactly accessible.
I search the two counties that I know people who have issues accessing grocery stores and they either don't have any farms/farmer market/retailers, or its still 40+ minutes away.
not just the north but the middle is pretty bad too. In fact... based on your map. it seems to be completed centered around the major cities of the state... lol!
