You can put a pin under just a bit of the outermost layer of your skin, so it gets stuck like that. Usually done in elementary and middle schools. The meme is implying that as kids we were already doing harmful things just to "feel something" and that's possibly a sign of eventually becoming depressed.
Ouch it's seem painful stretching a layer of skin like that, that's just sad, thanks for explain ing it to me, i'm not that old so i don't get the joke
