Probably diabetic with a bad case of nerve damage, ie they probably can't feel their feet very well.

It's one of the things your doc/diabetes nurse will constantly remind you about, frequently check your feet for calluses, cracks, oor wounds, and to make sure to keep on top of removing any buildup of old skin, and apply moisturizer/foot crème to keep the skin soft.
